Important : Cette décharge s’applique uniquement aux personnes non-membres de Houston Accueil, et exceptionnellement autorisées à participer à certaines activités ou événements. La participation à toute activité organisée par Houston Accueil est strictement conditionnée à la signature et à la remise de cette décharge.

Important: This waiver applies only to non-members, guests of Houston Accueil, and exceptionally authorized to participate in specific activities or events. Participation in any Houston Accueil activity, event, outing, or meeting is strictly conditioned on signing and submitting this waiver.

2. Décharge de Responsabilité / Waiver of Liability

Je, soussigné(e), participant(e) ou parent/tuteur du participant mineur indiqué ci-dessus, reconnais et accepte que ma participation (ainsi que celle de mon conjoint, enfant ou pupille) à toute activité, événement, sortie ou réunion organisée par Houston Accueil ou en son nom, est à mes propres risques.

I, the undersigned, participant or parent/guardian of the minor listed above, acknowledge and agree that my participation (and that of my spouse, child, or ward) in any activity, event, outing, or meeting organized by Houston Accueil or on its behalf, is at my own risk.

3. Indemnisation / Indemnification

Je m’engage à indemniser et défendre Houston Accueil, ses administrateurs, bénévoles et membres du bureau contre toute réclamation de tiers découlant de ma participation ou de celle de mon conjoint, enfant ou pupille.

I agree to indemnify, hold harmless, and defend Houston Accueil, its board members, officers, and volunteers against any claims from any person or entity arising from my participation or that of my spouse, child, or ward.

4. Autorisation Médicale pour Mineurs / Medical Authorization for Minors

En tant que parent ou tuteur, j’autorise Houston Accueil à fournir tout traitement médical ou dentaire d’urgence, y compris intervention chirurgicale si nécessaire, à mon enfant ou pupille pendant toute activité organisée ou sponsorisée par Houston Accueil.

As parent or guardian, I authorize Houston Accueil to provide any emergency medical or dental treatment, including surgery if necessary, for my child or ward during any activity organized or sponsored by Houston Accueil.
